However, it can be difficult to prove a violation of law or regulation. As a means to combat these claims, we offer a range of procedural tools. Those in a rush can use forms provided under OSC Circular D (for criminal claims). Complaints can be filed with OSC by usingĀ OSC Form-18. You may also accessĀ this and related forms from theĀ form menu when using the form browser in the OSC Website.Ā  We receive a substantial number of complaints regarding the use of OSC Circular D. In many cases that is because the complaint was handled by an Office of Special Counsel.

Violation of the Hatch Act (44 parts 658). This form is used by the OSC when requesting a response. The form must be completed in its entirety and must include the following information: Name of complaint; Title of complaint; Investigation, if any; Description of alleged violation; A statement that the investigation is initiated pursuant to section 7 of the Hatch Act and a description of the nature of the complaint. If a copy of a complaint or investigative report is also being submitted, please submit the form and a separate letter addressing the letter and report. In addition, OSC has provided a guide to use when reporting complaints in accordance with the Hatch Act. The letter must be sent to the appropriate OSC office. The letter must include: OSC's telephone number; To whom the letter may be addressed; and A description of the evidence or investigation supporting the grounds.
